To help provide the very best service, please use the autofill function on our requisitions or print all information legibly to ensure that your request is processed accurately.  Incomplete requisitions may delay the processing of testing and the subsequent reporting of results.  

A separate Requisition Form should be used for each patient and should be included with the accompanying sample.  Before shipping, please verify that patient information and patient ID number on the form matches that on the specimen.

Effective Monday, January 3, 2022, all final results for orders placed through OHSU's Clinical laboratories will be immediately released to OHSU MyChart. In order to block immediate release, the ordering provider should notify OHSU on the requisition form - at the time the order is placed - of their reasonable belief that an information blocking exception applies.

This change is being made to comply with Oregon Law and the Federal Information Blocking Rule.
