Background:

Numerical and structural chromosome changes are a recognized phenomenon in cancer progression.  Bone marrow chromosome analysis, as a companion to pathological assessment, may aid in the diagnosis and classification of malignant hematological disease.  The specific chromosome aberration, or lack thereof,  may be associated with prognosis and may be useful in directing therapy.   Continued monitoring of bone marrow chromosomes through treatment and remission allows assessment of disease status and the opportunity to observe clonal progression.  Companion fluorescent in situ hybridization (FISH) studies with appropriate probe sets are valuable in further delineation of chromosome abnormalities and in assessing minimal residual disease.

Methodology:

  • Chromosome analysis (karyotyping) with banding (<400-450 bands)
  • Reflex testing to FISH as appropriate to confirm diagnosis

Specimen Requirements:

Bone Marrow:  Shortly before aspiration add 0.2 cc of Sodium Heparin (1,000 units/ml) to tube of transport medium (please contact lab to arrange for transport medium to be sent).  Add at least 1 cc of bone marrow aspirate to the tube and invert several times to mix well.  Allow no clots to form.  It is preferable to send the first aspirate from the bone marrow draw.

  • After collection, keep at room temperature until shipping.

Bone Core Biopsy: may alos provide cells in cases where marrow is serverely packed. Send in transport medium with Sodium Heparin. Do not use fixative.
